Patient reported via consent form that he had not previously received any COVID vaccinations.
During the time he presented, the local county database was in downtime and we had to rely on the patient as a historian.
Since he stated he was unvaccinated at the time, he was administered Novavax COVID vaccine.
Database functionality has been restored and we have since discovered he previously received Pfizer COVID vaccination on 1/9/21, 2/4/21, and 9/17/21.
Patient was advised NOT to return to this pharmacy for a second dose of Novavax COVID vaccine.
